This is a question physicians face frequently, and its implications for patients, families, and the society are potentially huge. For many known conditions we have evidence, or guidelines to direct us. Where guidelines are lacking, common sense often leads us to an obvious solution. Either way, the worst way to deal with this issue, when it is present, is to ignore it.
